Principle of high frequency surgery

Monopolar electric scalpels pass an electric current through the animal between two electrodes: the active electrode and the passive Electrode (the neutral plate).
Standard bipolar forceps do not require a neutral plate, as the two tips are the two active electrodes.

The electrical signal is modulated by the machine depending upon the results you want to achieve:

  • Cutting
  • Coagulation
  • Cutting with coagulation

Monopolar function:
In monopolar mode, several types of current are used to:

  • Cut
  • Coagulate
  • Cut with haemostasis

Bipolar function:
The bipolar function is used for coagulation. It is very practical, in particular for the coagulation of blood vessels.
However, it is unsuitable for dissecting large tissue masses and ineffective for cutting.

The bipolar mode should be preferred to the monopolar mode due to the risk of burns relatede to:

  • high voltage 
  • poor contact with the neutral plate

Extra T.O.M™

Aspiration of surgical smoke and/or tissue irrigation

The aspiration of surgical smoke, aerosols and harmful gases directly from their source provides effective protection for the practitioner and his/her team,  and improved visibility of the surgical site.

Irrigation of the tissues makes gentle coagulation possible, without charring, for rapid control of complicated bleeding.

Functions with the blade electrode perpendicular to the matrix

  • Small cut (millimetre by millimetre): choose between 1 mm / 1.5 mm / 2 mm

Use the forceps in the closed position.
The blade electrode must be positioned in the matrix as follows:
Perform a movement with the entire forceps in the opposite direction to the blade.
No blade movement is required for this cut.
Use bipolar coagulation current or bipolar cutting current according to surgical need.

  • Large cut (up to 2 cm)

​Use the forceps in the closed position.
The blade elctrode must be positioned in the matrix as follows:
Perform a movement with the entire forceps in the opposite direction to the matrix.
No blade movement is required for this cut.Use the bipolar coagulation current or bipolar cutting current according to surgical need.

  • Cutting by gripping or opening (with the blade moving in the opposite direction)

Use the forceps with a closing or opening movement. 
The blade electrode must be positioned perpendicular to the matrix:
No forceps movement is required during this cut.

The blade moves when the forceps are closed or opened.
Use the bipolar coagulation current or bipolar cutting current according to surgical need.

  • Fine cut by gripping or opening (with the blade moving in the opposite direction)

Situational example​

Use the forceps by making a closing or opening movement.
The blade electrode must be positioned perpendicular to the matrix:
No forceps movement is required during this cut.
The blade moves when the forceps are closed or opened.
Use bipolar coagulation current or bipolar cutting current according to surgical need.

  • Closed forceps contact coagulation

Use the forceps in the closed position.
The blade electrode must be positioned in the matrix as follows:
No movement of the forceps is required during this coagulation.
No blade movement is required during this coagulation.
Use the bipolar coagulation current or bipolar cutting current according to surgical need.

  • Semi-open forceps contact coagulation (dedicated to large bleeds or for use of irrigation with the Extra T.O.M™ device

Use the forceps in the semi-open position.
The blade electrode must be positioned perpendicular to the matrix: 
No movement of the forceps is required during this coagulation.
No blade movement is required during this coagulation.
Use bipolar coagulation current or bipolar cutting current according to surgical need

Functions with the blade electrode flat on the matrix

  • Coagulation by grasping

​Situational example

Use closed forceps on tissue.
The blade electrode must be positioned flat on the matrix:
No movement of the forceps is required during this coagulation.
Close the blade flat against the tissue.
Use the bipolar coagulation current or bipolar cutting current according to surgical need.

  • Small lateral cut

Use the forceps in the closed position.
The blade electrode must be positioned flat on the matrix:
Perform a movement with the entire forceps in the opposite direction to the cut.
The forceps must be positioned so that the edge of the blade is in contact with the tissue.
No blade movement is required for this cut.
Use the bipolar coagulation current or bipolar cutting current according to surgical need.

  • End cut

Use the forceps in the closed position.
The blade electrode must be positioned flat on the matrix: 
Position the forceps at 90° to the direction of the cut.
Perform a movement with the entire forceps in the opposite direction to the cut.
No blade movement is required for this cut.
Use the bipolar coagulation current or bipolar cutting current according to surgical need.

  • Deep lateral cut

Use the forceps in the semi-open position.
The blade electrode must be positioned parallel to the matrix:
Perform a movement with the entire forceps in the opposite direction to the cut.
The forceps must be positioned so that the edge of the blade is in contact with the tissue.
No blade movement is required for this cut.
Use the bipolar coagulation current or bipolar cutting current according to surgical need.
